A few cents to a fraction of a dollar per transaction when bought directly from a clearinghouse, falling with volume; some sell monthly plans with a few hundred checks included. Most practices already pay for eligibility inside their EHR or clearinghouse contract and never see the per-check price. The check is cheap; interpreting the result is what costs staff time.
The transaction's low cost is why "we automate eligibility checks" is not much of a claim. The expensive part of verification was never the check. It is the reading, the follow-up and the phone calls that come after it, which is what the AI layer is priced for.
